how do u design a screen input? what does it look like?

this will depend on the question....
and looks like the way u want it to look like....my teacher said that u can draw if u want to fill spaces but this is nothing but wasting time
if it is for a hospital for example
it will have the patient number
patient room....a pic on the side
these kinda stuff....it just needs to be relevant and u must include essential stuff as the personal data and these kinda stuff

no you dont have to put a pic you just have to draw boxes & in each box write ex:
PATIENT RECORD
ID no.:
first name :
last name :
date of birth:
blood group:
address:
others:
FIRST NEXT LAST PRINT CLOSE

i didnt mean putting a REAL pic......anything i said put, its like virtual (specify spaces for them)
and By the way....i dont think the print and those things are required...

those are tabs whch are imp....dats wat our tchr said..
they hv to b put in boxes....
u cn put ny whch u think r appropriate nt necessary it has to b the same....
u cn also hv like:
Reset Submit etc....
it depends wat application form ur designing...
