Regarding the designing an input screen bit I think it has been phased out since 09, at least that's what my ICT teacher told me ...
The order does not matter, in order to attain full six points, you must include the book title and 4 optional items (data of issue, date of return, additional book details - synopsis ?, author name, genre, number in stock etc.), also the screen should look like an input screen, that means incorporating input mask and drop down list as required.

Yes, I am referring to the section which requires you to draw out the design for an input screen, it is no longer required.

do i have to put previous or forward button in output screen like records or in input screen only?
