To my knowledge, A* is awarded to the to top 10% or 13% of the students scoring an A. This happens in CIE GCE O Level and I think the same is the case with IGCSE.

An A* is calculated by adding up the grades of each INDIVIDUAL PAPER you answer.

Then it is determined if you fall into the A* band.
